Most of the systems we replace still work.
A national certification standard runs on a spreadsheet. An enterprise risk register lives in a folder of documents. An assessment that decides whether a building is compliant still arrives as a paper form.
None of it is broken, exactly. It is simply unmeasured — nobody can see the whole picture at once, nothing reconciles by itself, and every answer costs a person and an afternoon.
So the cost never appears on a report. It shows up as version confusion, as figures that disagree between two departments, as a process that only three people in the organisation actually understand.
